SPINE Primary Care Network (PCN) is a group of local doctors’ surgeries working together to help people stay healthy and well.
We team up with hospitals, social care, and local organisations to make sure care is joined up and easy to use. We focus on stopping problems early, giving quick support, and making services easier to reach.
Our goal is to build healthier neighbourhoods, help people stay well for longer, and give everyone a voice in their care.
